MHSS Eligibility Criteria

​

• Individual must be 18 years or older

• Must have a serious mental illness and or dual diagnosis of mental health and substance abuse which have resulted in significant impairment to daily activities

• Reentering the community from residential placement or psychiatric hospitalization

• Welfare has been abused/ threatened

• Mental, behavioral or emotional condition is such that it creates a major impairment in their ability to function in the home/community.
